INTRODUCTORY REMARKS ABOUT HOW WE DEAL WITH YOUR DATA:

The General Data Protection Regulation (GDPR) is a new law which will update and replace the current Data Protection Act. We are sure that many, if not all of you, have received a deluge of emails from a variety of organisations about GDPR - we certainly have!

Health Habitat always gathers, records and protects your personal information with care. Moreover, in doing so we want to make sure that all of our customers and subscribers are clear about what we do, how we do it, and that you have given us informed consent to record, use, and when applicable, store information to help us provide you with the service you have chosen.

We never sell to or share your data with any other third parties; we only use your data in order to provide you with the best service and support possible.

You are welcome to and for some services will be asked to, update or otherwise change the information we hold about you, at any time.

If you have subscribed to our mailing list, we will only store your data for as long as you choose to remain subscribed.

Customers who have purchased one of our products / services may be asked for more sensitive data. All data of a sensitive nature will be stored for the time period required by the nutritionist to analyse it and provide relative dietary and lifestyle advice, only.

Before customers partake in any one to one personal training sessions, they will be required to complete a physical activity readiness questionnaire (PARQ). The PARQ will be provided to the customer on paper and therefor must be completed using a pen. Some of this data is of a sensitive nature and every PARQ will be filed manually and stored securely. PARQ’s will be stored for twelve months and then removed/destructed. After twelve months clients will be required to complete a new form. Under no circumstances will any data collected via the PARQ form be converted/copied/transferred into a digital format.

You are welcome to both view or remove/destruct the data we have obtained from you at any time, providing it has not been removed/destructed already.

 

WHAT DATA DO WE COLLECT?

In the provision of our services, we may need to collect personal data from you. Personal data is any information that relates to a living, identifiable person. You have a right to know what data we collect and how it is used.

  • Waist Circumference (Optional)

  • Occupational Activity Level

  • Exercise Activity Level

  • Ante/Post Natal Status (If Applicable)

  • Dietary Goals

  • Food Allergies

  • Dietary Preferences (e.g. Vegan, Vegetarian, Halal & Kosher)

  • Meal Preferences

  • Current Drug/Supplement Protocols

 

We collect the following information about you when you complete the PARQ form to show your readiness regarding the one to one to one personal training sessions:

 

Contact Information

  • ​Name

  • Email Address

  • Emergency Contact Details

 

Sensitive Information

  • Date of Birth

  • A Brief History of Your Health (Relevant to the safety of the impending exercise participation).

  • Ante/Post Natal Information (If Applicable)

  • Your Signature

 

WHAT DO WE DO WITH COLLECTED DATA? 


Health Habitat both 'controls' and is responsible for how this data is processed, which includes collection, storage, use and removal/destruction of that data. 

The information we collect is essential to help us understand your needs, provide you with appropriate support/service and to monitor the progress made towards your goals.
